Malfunction in shearing operations at T1 facility

October 17, 1999
On October 17, 1999, following a malfunction in shearing operations at the T1 shearing-dissolution facility of the UP3 plant, old data was communicated to the facility control system. Based on measurements taken in the previous stage and consistent with that data, the dissolution process was allowed to continue.
Given the lack of any impact on operating personnel, the facility or the environment, a recommendation was made to the nuclear safety authority to classify this deviation from a requirement as a level 1 event on the International Nuclear Event Scale.
